2. Do you have an on-site clinic?
Yes. Our on-site clinic provides primary care services, allowing participants to receive medical attention conveniently in one location.

3. How is care coordinated?
Our interdisciplinary team works together to create a personalized care plan, ensuring each participant receives coordinated care across all medical, social, and support services.

4. Can I see specialists through Senior TLC?
Yes. If we don't provide it, we arrange access to needed specialists, including cardiology, orthopedics, and other disciplines, as part of your care plan.

5. How does Senior TLC’s medical care differ from regular primary care?
Unlike traditional care, Senior TLC’s PACE model integrates medical services with social support, transportation, and long-term care, giving participants a complete care solution.
